chứng hiển nhiên l{ có qu| nhiều
phương ph|p điều trị được đề xuất. Tại
sao vậy ???
Căn bản điều trị : hydroquinone, tr|nh
nắng, giảm tiếp x’c với oestrogen.
Kinh nghiệm điều trị : thường phối hợp
nhiều phương ph|p kh|c nhau.
Điều trị mạnh tay : chỉ đem lại hiệu quả
giới hạn1.
N|m t|i ph|t v{ kh|ng trị rất thường
gặp8 v{ chắc chắn sẽ gặp nếu không
chống nắng triệt để.

Sạm da nghịch thường paradoxical

Trong số c|c chế phẩm dạng thoa hiện nay được cho l{ điều trị n|m theo cơ chế
ức chế men tyrosinase, không sản phẩm n{o có chỉ định sử dụng kéo d{i trên BN
n|m m{ không g}y quan ngại về tính an to{n → Nhu cầu cần thiết phải tìm ra c|c
chất ức chế chuyên biệt trên men tyrosinase m{ có thể sử dụng thường quy v{
lâu dài.
Tất cả c|c phương ph|p điều trị n|m hiện nay đều g}y tổn thương da/viêm da
cho d‘ có biểu hiện l}m s{ng hay không.
Quan điểm cổ điển : Viêm = triệu chứng l}m s{ng : sưng, nóng, đỏ, đau
(iện nay : Viêm = tẩm nhuận tế b{o dòng bạch cầu v{/hoặc hóa chất đặc trưng
cho viêm. Quan điểm n{y bao gồm cả tình trạng viêm không được nhận biết bởi
BN lẫn BS l}m s{ng viêm không có biểu hiện l}m s{ng
Bản chất của phản ứng viêm : đ|p ứng sinh hóa của cơ thể với tình trạng tổn
thương hoặc tình trạng cơ thể tự coi l{ tổn thương v{/hoặc nhiễm tr‘ng.
Mục đích của phản ứng viêm : sửa chữa, tự vệ. Mặc d‘ mục đích n{y không phải
l’c n{o cũng đạt được, có khi còn g}y hại nếu phản ứng viêm không được cơ thể
kiểm so|t tốt.

sắc tố da có thể quan s|t thấy ngay sau tiến trình l{nh vết thương.
Hiện tượng viêm kín đ|o viêm dưới l}m s{ng thường không có biểu hiện sạm
da sau viêm nhưng lại l{m tăng hoạt hóa men tyrosinase trong một khoảng thời
gian rất l}u d{i sau khi phản ứng viêm không còn nữa. C|c men tyrosinase một
khi bị tăng hoạt hóa sẽ không thể trở về bình thường v{ có xu hướng l{m nặng
thêm tình trạng n|m da sau n{y.
Không riêng gì phản ứng viêm, tất cả c|c yếu tố g}y stress tế b{o đều khiến
melanocyte tăng sản xuất melanin v{ tăng sắc tố da32
Khi tế b{o bị tổn thương, DNA bị ph| hủy35,36 tạo th{nh c|c cyclobutane
pyrimidine dimers (CPDs) và thymidine dinucleotide TT, l{ th{nh phần chủ yếu

Sự hoạt hóa men tyrosinase30 :
– Sự tạo melanin phụ thuộc v{o hoạt tính của men tyrosinase hơn l{ tổng số

hóa tại vị trí của ph}n tử serin nằm trên chuỗi polypeptide nội bào.
– Phản ứng phosphoryl hóa được thực hiện qua trung gian của men protein
kinase C-beta (PKC-b). PKC-b được hoạt hóa bởi diacylglycerol có nguồn gốc
từ m{ng tế b{o bị tổn thương vì vậy m{ việc sử dụng topical corticoids để
ngừa P)( l{ một sự hợp lý : ngăn ngừa diacylglycerol, vừa ngăn ngừa
prostaglandin).

– Tế b{o sừng bị tổn thương sẽ b{i tiết một số hormone cận tiết để kích thích

lâm sàng).

Một hoạt chất có t|c dụng trị n|m tốt đòi hỏi phải thỏa m~n c|c đặc tính sau :
– Dễ d{ng thấm qua h{ng r{o da c‘ng với c|c t| dược ph‘ hợp
– T|c dụng chuyên biệt trên men tyrosinase
– Không g}y độc hoặc chết tế b{o
– Không g}y phản ứng viêm
C|c chế phẩm thoa đặc trị n|m hiện nay còn tồn tại một số khuyết điểm sau :
– Chất có hoạt tính trị n|m mạnh như hydroquinone thì g}y độc v{ g}y viêm
– Chất không g}y độc thì t|c dụng ức chế tyrosinase t|c dụng trị n|m yếu
– C|c chế phẩm phối hợp nhiều chất ví dụ (Q + RA + Steroids thì không thể
sử dụng kéo d{i
– Đa số kem chống nắng hiện nay không chống được tia UV-A

Bước đầu tiên trong qu| trình tổng hợp sắc tố melanin l{ sự oxy hóa Tyrosine
th{nh Dopaquinone dưới x’c t|c của men Tyrosinase. Đ}y l{ men quyết định tốc
độ phản ứng tạo melanin.
C|c bước còn lại xảy ra tự ph|t ở p( sinh lý tự oxy hóa : Dopaquinone chuyển
thành DOPA và Dopachrome.
DOPA cũng l{ cơ chất của Tyrosinase, v{ có thể bị oxy hóa trở lại th{nh
Dopaquinone.
Còn Dopachrome thì chuyển th{nh c|c hợp chất Indoles: Dihydroxyindole (DHI),
Dihydroxyindole-2-carboxylic acid D()CA , v{ c|c Quinones tương ứng
(Indolequinone (IQ) và Indolequinone-2-carboxylic acid (IQCA)).
C|c hợp chất indoles được tr‘ng hợp th{nh eu melanin
Quan điểm mới: DOPA thực ra không phải l{ chất trung gian được tạo ra từ
tyrosine, m{ thật ra nó được tạo ra sau n{y do sự khử nối đôi của Dopaquinone.

Trong chu trình monophenolase,
monophenol cụ thể l{ tyrosine chỉ phản
ứng với Eoxy v{ bị oxy hóa tạo th{nh oquinone. Eoxy trở th{nh Edeoxy để sẵn s{ng
gắn kết với O2 → t|i tạo lại Eoxy → kết th’c
chu trình monophenolase.
Trong chu trình diphenolase, nếu chỉ hiện
diện o-diphenols, cả dạng Eoxy và Emet đều
phản ứng với o-diphenol để oxy hóa nó
thành o-quinone. Cụ thể như sau:
Eoxy + diphenol → Emet + o-quinone
Emet + diphenol → Edeoxy + o-quinone
Như vậy, trong đa số trường hợp, luôn cần
thêm một ph}n tử diphenol để đưa
tyrosinase từ dạng Emet thành Edeoxy để có
thể tiếp tục qu| trình x’c t|c.
Vì vậy chu trình monophenolase luôn có
khoảng thời gian chờ lag time để có đủ
lượng diphenol cần thiết.

Effects of glyceollin on protein and mRNA expression of
melanogenic related genes
To characterize the expression of tyrosinase and melanogenic
related proteins, Western blot analysis was conducted using
specific antibodies. As shown in Fig. 3A, B16 melanoma cells
treated with glyceollin showed significantly decreased TRP-1,
MITF and tyrosinase protein expression levels when compared
with arbutin treated cells as a positive control; however, TRP-2
protein expression was not changed. In addition, an RT-PCR
assay of B melanoma cells treated with glyceollin and -MSH
was conducted to determine if glyceollin could also suppress the
mRNA level of the TRP-1, TRP-2, MITF and tyrosinase genes. As
shown in Fig. 3B, the tyrosinase level was significantly
decreased by glyceollin when compared with the control.
However, we did not observe a change in the TRP-1, TRP-2 and
MITF gene expression levels. These results suggested that
glyceollin may affect melanin synthesis through tyrosinase.
Glyceollin reduced melanogenesis through cAMP signaling
Because glyceollin decreased melanin synthesis and tyrosinase expression, we evaluated glyceollin to determine if it influenced the
expression of cAMP, which induces the expression of MITF, a master transcriptional regulator for melanogenic enzymes and tyrosinase
family proteins. To accomplish this, we evaluated glyceollin to determine if it reduces cAMP production in B16 melanoma cells. As
shown in Fig. 4A, we found that cAMP production decreased upon treatment with glyceollin in a dose dependent manner. Since we
established that glyceollin reduced the cAMP production in B16 cells, we investigated whether it is involved in the down-regulation of
MITF and tyrosinase expression. To accomplish this, we conducted western blot using forskolin, which is a cAMP-dependent protein
kinase A activator. As shown in Fig. 4B, the presence of glyceollin led to a significant decrease in the expression of MITF and tyrosinase.
Consistent with these findings, the tyrosinase activity induced by forskolin was suppressed by glyceollin (Fig. 4C), indicating the
involvement of cAMP signaling in glyceollin-inhibited melanogenesis. Collectively, our data indicate that glyceollin-reduced
melanogenesis is correlated with cAMP activation.

As Seed-ext was confirmed to potently
inhibit melanogenesis, activity guided
isolation of the active compounds was
carried out. Two lignans, , bisdemethylpinoresinol (1) and
americanin A (2), were isolated from
noni seeds and found to be active
constituents. As shown in Table 4, 10
and
μM of (1) displayed weak
inhibition of cell proliferation. But
1.25 to μM of (1) inhibited
melanogenesis in a concentration
dependent without any significant
effects on cell proliferation. Also, 100
and
μM of (2) inhibited
melanogenesis. (1) (IC50 value: 0.3
mM) and (2) (IC50 value: 2.7 mM)
exhibited tyrosinase inhibition, and
(1) (IC50 value:
μM and (2) (IC50 value:

exhibited potent DPPH radical
scavenging properties.

To investigate the effects of sulforaphane
on melanogenesis, the melanin contents
of the B16 cells without and with
treatment with sulforaphane were
measured. Arbutin was used as a positive
control. At doses of 0.1, 0.5, 1, 2.5, and 5
mM sulforaphane, melanin production
were compared with the untreated
control (Fig. 1). Sulforaphane decreased
melanin production 89.8 ± 3.2%, 82.4 ±
5.8%, 70.6 ± 11.8%, 48.5 ± 5.8%, and
35.9 ± 4.4% in a dose-dependent manner
without noticeable cytotoxicity up to a
concentration of 5 mM (Fig. 2). However,
there was a significant cytotoxic effect at
10 mM (data not shown). Nevertheless,
at 0.5 mM, sulforaphane significantly
decreased melanin production as
compared with the untreated control.
The inhibitory action of 5 mM
sulforaphane on melanogenesis was at a
level equivalent to that of 100 mM
arbutin.
